Health Declaration

In order to receive your runners kit you must sign the health declaration. If someone other than yourself is to pick up your kit for you, make sure that that person has the following items to submit: a photocopy of your ID card/passport, your health declaration signed by you which includes the Power of Attory. Only with these documents will the person be able to pick up the kit for you
